Nomination Form

This form is to be filled out by an LAI Minnesota Chapter Member. The Chapter Board will then review this application and approve the candidate, assuming they meet all qualifications. A nomination form must be filled out per potential nominee. 

Please inform nominees that LAI is a dues-based organization with an expectation of annual dues renewal to be a member in good standing. Email invoicing for the following year is facilitated each November by International. The yearly dues for members of LAI Minnesota Chapter are $290.

Once the nominee has been approved by the board, the sponsor and nominee will be notified, and an additional form will be sent to the nominee to complete.
